There's a lot of speculation about this subject. No one knows for sure since it was never clearly stated by Paul. It could be a number of things, but a lot of scholars and commentators have credited the thorn to either his bad eyesight, a disease of some sort, or his speech or lack thereof i.e maybe he stuttered.
Again we do not know because it is not clearly said in the Bible.
